Job summary

HCRS is seeking a Street Outreach Care Coordinator for Adult Services in Brattleboro, VT. This full-time role focuses on engaging individuals experiencing homelessness and housing instability in community settings, using trauma-informed, person-centered approaches to connect them with medical, behavioral health, housing, and social supports.

Qualifications

• Bachelor's degree preferred in social services or related field.
• At least 1 year of experience with vulnerable populations.
• Experience in outreach, case management or crisis intervention is an asset.
• Knowledge of trauma-informed care and housing resources beneficial.

Responsibilities

• Proactive outreach to individuals in streets, encampments, and nontraditional settings.
• Build rapport and engage clients who may be hesitant to access services.
• Develop individualized service plans with clients and coordinate care.
• Assist with identification, benefits, and housing resources.
• Coordinate referrals and warm handoffs with partners and providers.
• Document outreach activities and client progress in records.
