Job summary

Evergreen Treatment Services is seeking a Community Justice (LEAD) Case Manager to engage individuals suspected of low‑level offenses and homelessness, delivering intensive case management and outreach across Seattle’s Capitol Hill area.

You will assess housing and health needs, connect clients to benefits, and develop individualized service plans. Collaboration with LEAD partners and service providers is essential to advance recovery and community health.

Qualifications

• High school diploma or equivalent required; further education/training desirable.
• Experience engaging with people experiencing homelessness and co‑occurring disorders recommended.
• Ability to advocate and communicate under pressure in high‑stress situations.

Responsibilities

• Engage participants on the street and at service sites to build trust and offer services.
• Assess needs for housing, medical and mental health services; determine eligibility for funding (SSI, ABD, VA).
• Develop individualized service plans addressing food, shelter, health care, and recovery goals.
• Maintain relationships with LEAD partners, housing providers, and community programs to support participants.
• Provide advocacy within the criminal justice system, including court appearances and written communications.
• Attend team meetings and support coordinated efforts to improve community health and safety.
