Job summary

Qualifications

• Proficient in Word and Excel.
• Familiarity with EMR systems preferred.
• Practice Management experience preferred.
• Lean and project management experience preferred.

Responsibilities

• Meet regularly with team to monitor KPI’s and initiatives.
• Coordinate services across departments to ensure continuity of care.
• Conduct audits, assessments and performance reviews for improvement.
• Collaborate with clinicians to implement tailored care plans.
• Assist in budget planning and financial management with the clinical team.
• Manage provider panels, scheduling, and show rates to optimize access.
• Oversee patient onboarding and education related to Medical Home concepts.
