Job summary

Housing Forward in Dallas seeks a Navigation Coordinator to move individuals from navigator assignment to housing placement by coordinating System Navigators and internal teams across Dallas and Collin Counties. This role ensures timely housing outcomes through organized case management and strong cross-team collaboration.

You will lead regular meetings, track progress, and work with Landlord Engagement and Journey teams to resolve barriers, accelerating move-ins while maintaining accountability

Qualifications

• 2+ years in human services, housing services, or related fields.
• Experience coordinating services across multiple providers and managing caseload progress.
• Must have reliable transportation and a valid driver’s license.
• Must be able to work at different sites and lift up to 25 lbs.

Responsibilities

• Lead regular coordination meetings with contracted System Navigators to review households, housing progress, barriers, and next steps.
• Drive timely follow-through on housing plans, applications, documentation, and tasks to move households into housing.
• Track individual cases from navigator assignment through move-in, identifying delays and ensuring clear ownership of next steps.
• Escalate stalled cases to the Manager of System Navigation.
• Coordinate with Landlord Engagement, Journey, shelter providers, and housing programs to coordinate timely housing placements.
• Facilitate communication between navigators and Housing Forward teams to address barriers.
• Coordinate case conferencing for households needing additional support.
• Support consistent navigation workflows across Diversion, Transitional & Supportive Housing.
• Maintain regular communication with navigators and partner agencies regarding expectations and deadlines.
• Identify recurring challenges and propose operational improvements.
