You will effectively communicate and document any changes in patient conditions to the registered nurse or supervisor and maintain positive relationships with your patient and team. You'll be responsible for all practices and duties within the home health aide scope of practice as outlined by the state for its HHA personnel.**MAJOR AREAS OF RESPONSIBILITY:**Patient Care: Assist patients as needed with lifting and transferringCommunication: Maintain good relationships with the patient, the patient’s family, and staff.
