Job summary

Brentwood Hospital in Shreveport, Louisiana, a 260-bed psychiatric and chemical dependency treatment facility, seeks a dynamic Patient Navigator to support patients and families in achieving improved mental health outcomes.

The role involves guiding patients through the step-down process, discussing outpatient services (PHP/IOP/ECT), coordinating transportation, and ensuring timely discharge while reporting data to relevant departments and the CEO.

Qualifications

• Associates degree in nursing or bachelor’s in marketing/behavioral health required.
• Previous mental health experience preferred; must be competent in dealing with all aspects of the mental health field.
• RN or LPN licensure preferred.
• Hours: 8:30 AM - 5:00 PM, Monday - Friday.

Responsibilities

• Manages the workflow of the step-down process by identifying eligible patients for PHP/IOP/ECT before discharge.
• Meets with every eligible patient to discuss PHP/IOP/ECT or outpatient services, transportation, and obstacles to discharge within 24 hours of admission.
• Submits data to the PI committee and periodically reviews step-down navigation for process improvement.
• Independently plan and organize one's own schedule and activities.
