Job summary

Lighthouse Behavioral Health Hospital is seeking a Step-Down Navigator to support patients transitioning from inpatient care to outpatient services.

The role requires meeting with eligible patients, coordinating between departments, and communicating eligibility to senior leadership while maintaining data in the hospital systems. Minimum two years in inpatient psychiatric/mental health settings is expected; peer support preferred.

Qualifications

• Education: High School graduation or GED.
• Experience: Minimum two (2) years’ experience in an inpatient psychiatric/mental health setting.
• Licensure: Peer Support Certified preferred; CPR and Crisis Intervention Certification preferred.

Responsibilities

• Meet with eligible patients to discuss the benefits of step-down services as part of their continuum of care treatment.
• Communicate eligibility to senior leadership and associated departments.
• Coordinate between departments to ensure a smooth transition for patients.
• Conduct patient therapy groups.
