Job summary

Qualifications

• Master's degree in Social Work, counseling, or related mental health field from an accredited program.
• Must possess an Ohio-issued LSW, LISW, LPC or LPCC to qualify.
• Two years of experience in a mental health or substance abuse treatment setting with group therapy, crisis intervention, and treatment planning.

Responsibilities

• Meet eligible patients to discuss benefits of outpatient services as part of continuum of care.
• Routinely collect and enter outpatient eligibility data into Midas for trending reports.
• Understand the Step-Down Playbook and inpatient workflow as it relates to case management and discharge planning.
• Facilitate groups, assist with continued stay reviews and float to inpatient units as needed.
