Qualifications

• Master's degree in Social Work from an accredited college or university.
• Must possess OH LSW/LISW/LPC or LPCC licensure to qualify.
• Two years' experience in a mental health or substance use setting, with group therapy and crisis intervention.

Responsibilities

• Meet with eligible patients to discuss outpatient services as part of care.
• Communicate eligibility to senior leadership and relevant departments.
• Routinely collect and enter outpatient eligibility data into the Midas system.
• Understand the Step-Down Playbook and inpatient discharge workflow for case management.
• Facilitate groups and assist with continued stay reviews; float to inpatient units as needed.
