Job summary

UVA Health is seeking a Registered Nurse Care Coordinator for the Stem Cell Transplant Clinic in Charlottesville. You will guide patients through complex treatment journeys, coordinating care from referral to post-transplant follow-up and acting as a trusted clinical guide for families.

Ideal candidates have strong communication and organizational skills, a collaborative mindset, and the ability to manage multiple priorities in a fast-paced ambulatory setting.

Qualifications

• Bachelor of Science in Nursing from an accredited nursing program.
• 1 year of registered nursing experience is required; 3 years strongly preferred.
• Licensed to Practice as a Registered Nurse in Virginia; AHA BLS required.

Responsibilities

• Coordinate care for adult stem cell transplant and cellular therapy patients from referral through post-transplant follow-up.
• Serve as primary point of contact for patients and families, providing education, guidance, and support.
• Manage transplant timelines and ensure milestones are completed accurately and efficiently.
• Collaborate with physicians, advanced practice providers, pharmacists, social workers, and other team members.
• Facilitate communication across departments to support coordinated, patient-centered care.
