Job summary

UVA Health seeks a Registered Nurse Care Coordinator for the Stem Cell Transplant Clinic in Charlottesville, VA. You will guide patients through stem cell transplant and cellular therapy, coordinating care from referral to post-transplant follow-up with empathy, education, and multidisciplinary collaboration.

You will work with a specialized transplant team in a fast-paced ambulatory setting, building trusted relationships with patients, families, and colleagues while managing complex treatment

Qualifications

• Bachelor of Science in Nursing from an accredited program.
• 1 year of RN experience required; 3 years of related RN experience strongly preferred.
• Virginia RN license and AHA Health Care Provider BLS certification required.
• Specialty board certification is strongly preferred.

Responsibilities

• Coordinate care for adult stem cell transplant and cellular therapy patients from referral through post-transplant follow-up.
• Serve as primary contact for patients and families, providing education, guidance, and support.
• Manage transplant timelines and ensure critical milestones are completed accurately and efficiently.
• Collaborate with physicians, advanced practice providers, pharmacists, social workers, and other multidisciplinary team members.
• Facilitate communication across departments to support safe, coordinated, patient-centered care.
