Job summary

UnitedHealth Group's Optum HouseCalls team seeks an Advanced Practice Clinician (NP or PA) to conduct annual in‑home health assessments for Medicare Advantage and other plan members. This field-based role is non-prescribing, emphasizes education, gap closure, and collaboration with primary care providers.

Flexible scheduling (8am–8pm, 7 days) with ~85% travel across New York, travel expenses covered. You’ll be part of an interdisciplinary care environment, helping patients access and navigate

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Current NCCPA certification and state licensure
• Active, unrestricted licensure in the practicing state by start date
• Driver's license and reliable transportation for home visits
• Ability to navigate varied home environments during assessments

Responsibilities

• Conduct comprehensive in-home assessments, including past medical history, med reconciliation, vitals, and physical exam
• Identify and document diagnoses for care management and treatment planning
• Communicate findings to PCPs to address gaps in care
• Recognize urgent/emergent situations and intervene appropriately
• Educate members on disease processes, medications, and adherence
• Address social determinants of health and provide referrals as needed
