Job summary

UnitedHealth Group’s Optum HouseCalls team seeks an Advanced Practice Clinician (NP/PA) to perform in-home health assessments for Medicare Advantage and other members. This field-based role emphasizes education, gap closure, and collaboration with primary care providers to improve health outcomes.

Qualifications

• Active ANCC or AANP certification in Family/Adult/Geriatric Primary Care.
• NCCPA certification and state licensure for PAs.
• Active, unrestricted licensure in the state you will practice in with no disciplinary actions.
• Active and unrestricted driver’s license with reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments.

Responsibilities

• Conduct comprehensive in-home assessments including medical history, meds reconciliation, vital signs, and exam.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to members’ PCPs to address gaps in care.
• Recognize urgent situations and intervene appropriately.
• Educate members on diseases, medications and adherence.
• Address social determinants of health and provide referrals as needed.
