Job summary

UnitedHealth Group's Optum HouseCalls program seeks Advanced Practice Clinicians (NPs or PAs) to conduct in-home health assessments for Medicare Advantage members across CT and RI. This is a field-based role with travel throughout the states and no on-call requirements.

The position offers flexible provider schedules (20–40 hours weekly), travel expense coverage, and a robust benefits package. Sign-on and loan repayment options are available for eligible applicants.

Qualifications

• NPs: ANCC/AANP family/adult/geriatric certification required.
• PAs: NCCPA certification and state licensure required.
• Active, unrestricted licensure in practicing state.
• Driver's license and reliable transportation for home visits.
• Bilingual in Spanish.

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage members.
• Document diagnoses for care management and treatment planning.
• Communicate findings to PCPs and address care gaps.
• Recognize urgent/emergent situations and intervene.
• Educate members on disease processes, medications, and compliance.
• Address social determinants of health and provide referrals.
