Job summary

Optum Home & Community Care is seeking an Advanced Practice Clinician (NP or PA) to perform comprehensive in-home health assessments for Medicare Advantage and other plan members.

This field-based role emphasizes education, gap closure, and collaboration with primary care providers, with travel required about 85% of the time across multiple states. Flexible scheduling and a balanced work-life approach are offered.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted licensure in the state you are practicing in without current disciplinary actions or practice limitations.
• 1+ years of clinical experience (family, geriatric, or home health preferred).
• Active and unrestricted driver's license.
• Able and willing to travel via airplane majority of the time.

Responsibilities

• Conduct in-home assessments for Medicare Advantage and other plan members.
• Perform evidence-based screenings and point-of-care testing as appropriate.
• Document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to address gaps in care.
