Job summary

The Ohio Adult Care Facilities Association is seeking a Housing Navigator (Case Manager) to support individuals with SPMI statewide in a supported housing program. You will assess needs, develop service plans, and guide clients through housing options, while coordinating with home operators and community partners.

Ideal candidates have a relevant bachelor’s degree and extensive case management experience, plus strong communication, advocacy, and record-keeping skills.

Qualifications

• Bachelor’s degree in social work, human services, public health, or related field with 5 years of case management or a master’s degree with 2 years of case management experience.
• Strong knowledge of statewide housing programs, community resources, social service systems, Medicaid programs, and benefits.
• Experience with individuals with complex needs such as mental illness or substance use disorders, including homelessness or crises.
• Excellent case management skills: interviewing, assessment, planning, crisis intervention, and de-escalation.
• Outstanding communication and interpersonal skills; ability to work independently and in a team.
• Proficient in data entry and maintaining accurate records; experience with diverse populations.

Responsibilities

• Assess client needs and functioning to determine service eligibility based on rules and protocols.
• Conduct comprehensive assessments of housing, mental health, substance use, medical, financial, and supportive needs.
• Develop and implement personalized service plans to achieve housing goals.
• Coordinate with partners to connect clients to services addressing medical, behavioral health, and social needs.
• Assist clients with locating Residential Facilities Class 2 or 3 and manage housing applications and move-in processes.
• Advocate for clients with government entities and community organizations to promote stability and access to services.
• Maintain accurate records and progress notes in program systems; complete reporting requirements.
