Recovery and Wellness Centers of Midwest Ohio in Tipp City, OH seeks a Case Manager to coordinate services for clients with SPMI/PMI, collaborate with a multidisciplinary team, and ensure timely documentation. The role emphasizes crisis support, client advocacy, and resource linkage within a community setting.

Requires a 4-year degree (or equivalent), 3+ years of relevant experience, or an associate degree with experience, plus a valid driver's license and strong communication skills.

Qualifications

• A 4 year degree or higher from an accredited institution and 3+ years of experience working with SPMI/PMI preferred.
• Associate’s degree in a Social Services or Psychology related field with less than 3 years experience may be considered.
• Valid driver’s license and appropriate automobile insurance required.

Responsibilities

• Informs clients of scope of practice, supervision status, and describes supervisor details.
• Performs limited assessment and coordinates with supervisor to implement treatment plans.
• Maintains efficient clinical documentation within 24 hours of services provided.
• Coordinates services with a multi-disciplinary team and provides crisis support as needed.
• Advocates for client rights and helps access community resources.
