National Church Residences seeks a Case Manager I in Columbus, OH to provide direct case management for Permanent Supportive Housing residents using a Housing First approach. You will work with an Integrated Primary and Behavioral Health Team to connect residents with resources and services.

A bachelor's degree in social work or related field is required, with 1 year of human services experience preferred. The role involves monthly client contact, documentation, group facilitation, and adherence

Qualifications

• Bachelor's degree in Social Work or a related field is required.
• 1 year of experience in human services preferred.
• Licensure as SW or LPC preferred; valid driver's license required.

Responsabilités

• Conduct bio-psychosocial assessments and develop strengths-based care plans.
• Monitor and follow up with clients on service needs.
• Assist clients with benefits applications and accessing services.
• Document encounters in electronic records within required timeframes.
• Facilitate weekly psychosocial and educational groups.
• Maintain regular caseload contact and transportation to appointments as needed.
• Attend weekly blended management meetings and advocate for clients.
• Maintain compliant client files and complete training and development.
