YAI is seeking a qualified START Coordinator to manage an active caseload of individuals needing START services and to coordinate comprehensive care plans across clinical and community settings.

The role requires a Master’s degree in a human services field, 2+ years of I/DD or behavioral health experience, and START Coordinator Certification within 12 months. Travel and on-call duties are essential aspects of the position.

Formación

• Master's degree in Mental Health, Psychology, Counseling, Social Work or other Human Service field.
• At least two (2) years of experience working with the I/DD population and/or behavioral health needs.
• Successful completion of START Coordinator Certification training within 12 months of appointment.

Responsabilidades

• Maintains an active caseload of up to 25 individuals referred for START Services.
• Conducts outreach visits to individuals' homes, day, and vocational settings as needed.
• Coordinates support meetings and crisis plan development for START clients.
• Develops Comprehensive Service Evaluations and related plans for START and Resource Center admissions/discharges.
• Completes and maintains all required documentation and data entry for START systems.
• Provides on-call support on a rotating basis as part of the START team.
