Job summary

YAI is seeking a START Coordinator to manage a caseload of individuals receiving START services, performing assessments, and coordinating care across home, community, and clinical settings.

The role requires a Master’s degree in a human services field, two years’ experience with I/DD or behavioral health, and willingness to travel. Certification within 12 months is expected, with on-call coverage and robust documentation duties.

Qualifications

• Master’s degree in Mental Health, Psychology, Counseling, Social Work or related Human Service field.
• At least two years of experience with I/DD and/or behavioral health needs.
• Satisfactory combination of education/experience per START/NCSS; bachelor’s degree required for START Coordinators.
• Completion of START Coordinator Certification training within 12 months.
• Ability to provide on-call crisis coverage regularly as designated by START leadership.
• Ability to travel regularly within assigned region and to other states as needed.
• Ability to meet essential physical demands including lifting up to 25 pounds.
• Strong interpersonal and problem-solving skills; independent and organized with good communication.
• Willingness to take initiative and be flexible.
• For California postings: valid Driver’s License

Responsibilities

• Maintains an active caseload of up to 20–25 individuals referred for START Services and provides systemic consultation.
• Conducts regular outreach visits to individuals’ home, day, or vocational settings as required.
• Assists with referrals for enrollment in START and completes intake assessments.
• Coordinates meetings and crisis plans for START participants and communicates with leadership.
• Develops Comprehensive Service Evaluations and plans for START services.
• Participates in consultations and meetings with START leadership and clinical teams.
• Provides on-call support on a rotating basis.
• Completes and maintains all documentation timely.
• Enters data into START Reporting System and other required systems.
• Maintains outreach with community partners and presents at Clinical Education Team Meetings.
• Participates in the Center for START Services National Network and related events.
• Completes trainings to ensure compliance with YAI policies and regulatory requirements.
• Performs other duties as assigned.
