YAI - Seeing beyond disability in Bronx is seeking a START Coordinator to manage a caseload of individuals and provide system-level consultation. The role emphasizes home and community outreach, intake and enrollment tasks, and crisis planning within a multidisciplinary START team.

Qualified candidates have a Master’s degree in a human service field, two years’ experience with I/DD or behavioral health needs, and must obtain START certification within 12 months.

Qualifications

• Master’s degree in Mental Health, Psychology, Counseling, Social Work or other Human Service field.
• At least two years of experience working with the I/DD population and/or behavioral health needs.
• Completion of START Coordinator Certification training within 12 months of appointment.
• Ability to provide on-call crisis coverage on a regular basis, including evenings, weekends, and overnights.
• Ability to travel regularly within the assigned region and to other states for NCSS meetings or events.

Responsabilités

• Maintains an active caseload of up to 20–25 individuals referred for START Services and provides systemic consultation.
• Conducts regular outreach visits to individuals’ homes, day and vocational settings, in-person or via telehealth.
• Coordinates enrollment, intake assessments and referrals to clinical teams; develops crisis plans and START action plans.
• Participates in coordination meetings, trainings, and national START network activities; maintains required documentation and data entry.
