Job summary

YAI - Seeing beyond disability seeks a START Coordinator to manage a caseload of 20–25 individuals, provide systemic consultation, and conduct home and telehealth assessments. Qualifications

• Master’s degree in Mental Health, Psychology, Counseling, Social Work or other Human Service field.
• At least two years of experience with I/DD and/or behavioral health needs.
• START Coordinator Certification training within 12 months of appointment.

Responsibilities

• Maintains an active caseload of 20–25 individuals referred for START Services.
• Provides systemic consultation to systems/teams using best practices.
• Conducts home visits and telehealth evaluations as needed.
• Assists with referrals and intake assessments across regions.
• Coordinates support meetings and crisis plans; escalates concerns as needed.
• Develops evaluations, intervention and outcome plans for START programs.
• Participates in regular leadership meetings and on-call rotation.
• Completes documentation and data entry in START systems on time.
• Travels regularly to conduct in-person assessments; may travel to other states for NCSS events.
