Job summary

YAI is seeking a START Coordinator to support individuals with I/DD and co-occurring behavioral health needs in the East Syracuse area. You will evaluate, assess, and coordinate with the interdisciplinary team to ensure fidelity to the START model and provide on-call support.

The role requires a master’s degree in a human services field, two years of relevant experience, and START Coordinator Certification within 12 months. Travel and telehealth options are involved as needed.

Qualifications

• Master’s degree in Mental Health, Psychology, Counseling, Social Work, or related field.
• At least two years of experience with I/DD population and/or behavioral health needs.
• Completion of START Coordinator Certification training within 12 months of appointment.

Responsibilities

• Maintain an active caseload of 20–25 individuals referred for START services.
• Conduct in-person and telehealth evaluations and assessments.
• Coordinate referrals, enrollments, and intake across regions as needed.
• Organize and participate in crisis planning and team meetings for START.
• Enter data into START Information Reporting System and other required systems.
