Job summary

YAI - Seeing beyond disability is hiring a START Coordinator in Watsonville, CA. The role involves managing a caseload of 20–25 individuals and providing comprehensive START services with regular outreach, intake, and crisis planning.

You will coordinate with clinical leads, document all activities, and participate in ongoing training. The ideal candidate has a Master’s in a human services field, a Bachelor’s degree, and the START Coordinator Certification within a year.

Qualifications

• Master’s degree in Mental Health, Psychology, Counseling, Social Work or other Human Service field.
• Bachelor’s degree in human services is preferred for START Coordinators.
• Successful completion of START Coordinator Certification training within 12 months of appointment.

Responsibilities

• Maintains an active caseload of up to 20–25 individuals referred for START Services and provides systemic consultation.
• Conducts regular outreach visits to individuals’ homes, day and/or vocational settings (in-person or telehealth).
• Assists with referrals for enrollment in START and completes intake assessments.
• Coordinates support meetings and crisis plan development; relays information to Medical/Clinical Directors.
• Develops Comprehensive Service Evaluations and treatment plans for START programs; documents services provided.
• Participates in regular START leadership and clinical team meetings; provides on-call support on a rotating basis.
• Completes documentation and data entry into START Information Reporting System and other systems; ensures compliance with policies.
• Maintains partnerships with community organizations and presents at Clinical Education Team Meetings.
