Qualifications

• Current CNA license or higher licensure.
• High School Diploma or GED required.
• Minimum two years administrative experience; one-year staffing and/or caregiving preferred.

Responsibilities

• Monitor completion of home care tasks and shifts; report concerns to Home Care Manager.
• Oversee staffing process; develop/update Home Care schedule one month in advance; coordinate referrals.
• Ensure participant home care schedules reflect IDT plan of care.
• Develop and maintain rapport with Home Care Staff and coordinate schedules.
