Job summary

Volunteers of America National Services is seeking a Registered Nurse in Lanham, MD to deliver compassionate nursing care within the PACE program. This on-site role focuses on initial assessments, care planning, and coordinating with an Interdisciplinary Team to ensure comprehensive 24-hour care for participants.

The ideal candidate will have at least one year of experience with frail or elderly populations, hold a valid RN license, CPR certification, and a driver's license with reliable

Qualifications

Minimum of one year’s experience with frail or elderly populations.
Graduate of an accredited school of professional nursing.
Current state RN license and CPR certification.
Valid driver’s license and reliable transportation.

Responsibilities

Conducts in-person nursing assessments in the PACE clinic and participants' homes to develop a plan of care.
Performs semi-annual re-assessments and participates in on-call rotation.
Collaborates with Interdisciplinary Team to coordinate 24-hour care delivery.
