Job summary

The Senior Community Care of Maryland PACE, a VOANS program, seeks a Registered Nurse in Lanham, MD to perform initial assessments, develop care plans, and coordinate with an interdisciplinary team in a community-based PACE setting.

Requirements include an RN license, CPR certification, and at least one year of geriatric care experience. A valid driver’s license and transportation are also required. Enjoy comprehensive benefits and growth opportunities in a supportive environment.

Qualifications

• A minimum of one year’s experience in working with the frail or elderly population.
• Current state Registered Nurse (R.N.) License, CPR certification required.
• Must have a valid driver’s license, proof of insurance and have means of transportation.

Responsibilities

• Conducts an in-person comprehensive initial nursing assessments in the PACE clinic and in participant’s homes with new PACE enrollees. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
• Conducts in-person nursing re-assessments in the PACE clinic and in participant’s homes semi annually and as needed.
• Participates in the nursing on-call rotation.
• Functions as a member of the Interdisciplinary Team and participates in Interdisciplinary Team meetings; communicates participant changes and coordinates twenty-four (24) hour care delivery.
