Qualifications

Job Responsibilities
  1. Process agency home health referrals by reaching out to clinicians accurately and promptly.
  2. Collaborate with team members to ensure timely Start of Care and Resumption of Care for all patients.
  3. Track and follow up with patients pending start of care.
  4. Report all non-admitted patients to the Intake Supervisor/Administrator.
  5. Serve as the liaison between the company and patients/caregivers.
  6. Engage with referral sources and the sales team regarding new referrals.
  7. Understand and effectively communicate all programs offered by the company.
  8. Learn and retain medical terminology and protocols.
  9. Manage electronic filing.
  10. Perform all other duties assigned by the supervisor.
Qualifications/Skills
  1. Strong written and verbal communication skills.
  2. Flexibility and a cooperative attitude.
  3. Excellent customer service skills.
  4. Detail-oriented with organizational skills.
  5. Ability to work independently and in a team environment.
  6. Proficiency in spreadsheet preparation and tracking.
  7. Presentation and administrative reporting skills.
  8. Proficient in Microsoft Office Suite.
  9. Accurate documentation skills.
  10. Ability to manage multiple demands and multitask effectively.
Education and Experience Requirements
  1. Associates Degree preferred; Bachelor’s degree or work equivalent.
  2. At least one year of experience in home health care is preferred.
  3. One year of administrative support experience.
  4. Familiarity with medical terminology is preferred.
  5. Scheduling experience.
