Job Responsibilities:

  • Process agency home health referrals by reaching out to clinicians in an accurate and timely manner.
  • Work in collaboration with all members of the pod to ensure times Start of Care and Resumption of Care for all patients.
  • Track and follow up with all patients who are pending start of care.
  • Report all non-admit patients to the Intake Supervisor/Administrator.
  • Serve as the liaison between the company and patients/caregivers.
  • Interact with referral sources and the sales team regarding new referrals.
  • Understand and can effectively explain all programs offered by the company.
  • Ability to learn and retain medical terminology and protocols.
  • Electronic filing
  • All other duties assigned by supervisor.

Education and Experience Requirements:
  • Associates Degree Preferred/Bachelor’s degree and/or work equivalent
  • At least one (1) year of experience in home health care is preferred.
  • One year of administrative support experience
  • Familiar with medical terminology is preferred
