Job summary

Chandler Creek Post Acute in Greer, SC is seeking a full-time Speech-Language Pathologist (SLP) to evaluate, treat, and support residents with speech, language, cognitive, and swallowing disorders. You’ll collaborate with physicians, nursing, and therapy staff to deliver patient-centered care.

The role requires SC licensure; CCC-SLP is preferred, with experience in nursing or rehab settings. Competitive pay and comprehensive benefits accompany a supportive rehab team.

Qualifications

• Master's or Doctoral degree in Speech-Language Pathology
• Active, unencumbered SLP license in South Carolina (required)
• CCC-SLP preferred
• Experience in a skilled nursing or rehab setting preferred
• Knowledge of speech, language, cognitive, and swallowing disorders
• Experience with PCC and Casamba preferred
• Strong clinical, communication, and documentation skills
• Ability to work both independently and as part of a team

Responsibilities

• Perform evaluations within 24 hours of physician referral
• Develop and implement individualized treatment plans with physician approval
• Provide therapy services in accordance with treatment plans
• Monitor patient progress and adjust care as needed
• Maintain accurate clinical documentation (evaluations, progress notes, discharge planning)
• Participate in care plan meetings and interdisciplinary conferences
• Educate residents, families, and staff on therapy programs and techniques
• Supervise SLPAs and CFYs as applicable
• Conduct staff training and in-service education
• Ensure compliance with all regulatory and facility standards

Position Summary

As a Speech-Language Pathologist (SLP), you will evaluate, treat, and support residents with speech, language, cognitive, and swallowing disorders. You'll collaborate closely with physicians, nursing, and therapy teams to deliver individualized, patient-centered care.

Key Responsibilities

• Perform evaluations within 24 hours (or 1 business day) of physician referral


• Develop and implement individualized treatment plans with physician approval


• Provide therapy services in accordance with treatment plans


• Monitor patient progress and adjust care as needed


• Maintain accurate clinical documentation (evaluations, progress notes, discharge planning)


• Participate in care plan meetings, utilization review, and interdisciplinary conferences


• Educate residents, families, and staff on therapy programs and techniques


• Supervise SLPAs and Clinical Fellows (CFY) as applicable


• Conduct staff training and in-service education


• Ensure compliance with all regulatory and facility standards

Qualifications

• Master's or Doctoral degree in Speech-Language Pathology
