Job summary

Chandler Creek Post Acute in Greer, SC is seeking a dedicated Speech-Language Pathologist (SLP) to join our full-time rehab team. You will evaluate, treat, and support residents with speech, language, cognitive, and swallowing disorders.

Collaborate with physicians, nursing, and therapy staff to deliver patient-centered care. The role emphasizes timely evaluations, evidence-based treatment plans, accurate documentation, and supportive supervision of SLPAs as applicable.

Qualifications

• Must have Master’s or Doctoral degree in Speech-Language Pathology.
• Active, unencumbered SLP license in South Carolina.
• CCC-SLP preferred credential.

Responsibilities

• Perform evaluations within 24 hours (or 1 business day) of physician referral.
• Develop and implement individualized treatment plans with physician approval.
• Provide therapy services in accordance with treatment plans.
• Monitor patient progress and adjust care as needed.
• Maintain accurate clinical documentation (evaluations, progress notes, discharge planning).
• Supervise SLPAs and CFY as applicable.
• Ensure compliance with regulatory and facility standards.
