Job summary

Lakeport Post Acute in Lakeport, CA is hiring Speech-Language Pathologists (SLPs) to help residents regain communication, swallowing, and cognitive skills. Salary ranges from $65.00 to $70.00 per hour DOE, with sign-on bonus and relocation assistance.

Both full-time and part-time opportunities exist, with flexible scheduling and a supportive interdisciplinary team. The ideal candidate holds a Master’s in Speech-Language Pathology, CA licensure, and CCC-SLP (or eligibility).

Qualifications

  • Master's degree in Speech-Language Pathology required.
  • California SLP license current.
  • CCC-SLP preferred or eligible to obtain.
  • CF candidates are encouraged to apply.
  • Experience in SNF or post-acute care settings preferred.

Responsibilities

  • Evaluate and treat residents with speech, language, voice, cognitive-communication, and swallowing disorders.
  • Develop and implement individualized treatment plans based on resident goals and clinical needs.
  • Conduct dysphagia assessments and provide evidence-based swallowing interventions.
  • Treat residents with neurological, cognitive, and medically complex conditions.
  • Collaborate with physicians, nursing staff, rehabilitation professionals, residents, and families.
  • Educate residents, caregivers, and staff regarding treatment recommendations and compensatory strategies.
  • Participate in interdisciplinary care planning and discharge planning activities.
  • Maintain accurate, timely documentation in accordance with facility, state, federal, Medicare, and reimbursement requirements.
  • Monitor patient progress and adjust treatment plans to maximize outcomes.
  • Support resident safety, communication, nutrition, and overall quality of life.

Speech-Language Pathologist (SLP) Responsibilities
  • Evaluate and treat residents with speech, language, voice, cognitive-communication, and swallowing disorders

Qualifications
  • Master's Degree in Speech-Language Pathology required
  • Current California Speech-Language Pathologist License required
  • Certificate of Clinical Competence (CCC-SLP) preferred or eligibility to obtain
  • Strong clinical, communication, and interpersonal skills
  • Compassionate, patient-centered approach to care
  • Ability to work effectively within an interdisciplinary healthcare team
  • Experience in Skilled Nursing Facility (SNF), Post-Acute Care, Rehabilitation, Long-Term Care, or Acute Care settings preferred
  • Knowledge of dysphagia management, modified diets, and swallowing disorders preferred
  • Experience with cognitive, neurological, and medically complex patient populations preferred
