Job summary

Floresville Nursing and Rehabilitation seeks an experienced Speech-Language Pathologist to develop and implement therapy plans for residents and short-term patients in a skilled nursing setting.

You will screen patients as needed, track progress with the team, and document care and outcomes in a timely, compliant manner while maintaining a safe, well-equipped rehab area. This full-time role offers strong support and opportunities for licensure reimbursement.

Qualifications

• Current Texas licensure as an SLP is required.

Responsibilities

• Develop and implement a Speech Therapy plan of care for residents and short-term patients.
• Assist with patient screens for Speech Therapy as needed.
• Communicate progress and barriers toward SLP goals with the interdisciplinary team.
• Complete timely Speech therapy documentation in compliance with regulatory, licensing, facility, and payor requirements.
• Maintain rehab gym equipment and treatment area in a safe manner with infection prevention methods.
• Provide and document training for patient, family, and caregivers regarding the plan of care.
• Maintain productivity standards as directed by the Director of Rehab or Administrator.

Job description

This position is for a skilled nursing facility in Floresville. Floresville Nursing and Rehabilitation has both short-term and long-term patients. This facility has an in-house therapy team with smaller daily case loads withan emphasis on optimizing time with patients. We have flexible scheduling!

Speech Therapist Job Responsibilities

• Develop and implement a Speech Therapy plan of care for facility residents and short-term patients.
• Assist as needed with patient screens for Speech Therapy.
• Communicate progress and any barriers toward SLP goals with interdisciplinary team members.
• Complete timely Speech therapy documentation in compliance with regulatory, licensing, facility, and payor requirements.
• Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods.
• Provide and document patient, family, and caregiver training regarding each speech therapy patient’s specific plan of care and recommendations for follow up.
• Maintain productivity standards as expected by Director of Rehab or Administrator.
