Job summary

Harbor Point Skilled Nursing is seeking a Speech Therapist to develop and implement a plan of care for residents and short-term patients in a skilled nursing setting. You will screen patients, communicate progress to the interdisciplinary team, and document therapy activities in compliance with regulations.

Current Texas licensure as an SLP is required; new grads are welcome to apply. Schedule is full-time, with a salary from $45.00 per hour.

Responsibilities

• Develop and implement a Speech Therapy plan of care for facility residents and short-term patients.
• Assist as needed with patient screens for Speech Therapy.
• Communicate progress and any barriers toward SLP goals with interdisciplinary team members.
• Complete timely Speech therapy documentation in compliance with regulatory, licensing, facility, and payor requirements.
• Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods.
• Provide and document patient, family, and caregiver training regarding each speech therapy patient’s specific plan of care and recommendations for follow-up.
• Maintain productivity standards as expected by the Director of Rehab or Administrator.
