Job summary

Harbor Point Skilled Nursing seeks a Speech-Language Pathologist to develop and implement therapy plans for both long‑term and short‑term residents. The role includes patient screens, progress communication with the interdisciplinary team, and thorough documentation to meet regulatory and facility standards. New graduates are welcome.

Responsibilities

• Develop and implement a Speech Therapy plan of care for residents and short-term patients.
• Assist with patient screens for Speech Therapy.
• Communicate progress and barriers toward SLP goals with interdisciplinary team members.
• Complete timely Speech therapy documentation per regulatory and facility requirements.
• Maintain rehab gym equipment and treatment area with infection prevention.
• Provide and document patient, family, and caregiver training on each plan of care.
• Maintain productivity as directed by Director of Rehab or Administrator.

This position is for a skilled nursing facility with both short-term and long-term patients. Harbor Point Skilled Nursing has an in-house therapy team with smaller daily case loads, all new equipment, and an emphasis on optimizing time with patients.

Speech Therapist Job Responsibilities:

• Develop and implement a Speech Therapy plan of care for facility residents and short-term patients.

• Assist as needed with patient screens for Speech Therapy.

• Communicate progress and any barriers toward SLP goals with interdisciplinary team members.

• Complete timely Speech therapy documentation in compliance with regulatory, licensing, facility, and payor requirements.

• Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods.

• Provide and document patient, family, and caregiver training regarding each speech therapy patient’s specific plan of care and recommendations for follow-up.

• Maintain productivity standards as expected by the Director of Rehab or Administrator.
