Job summary

Harbor Point Skilled Nursing in Texas is seeking a Speech-Language Pathologist to develop and implement therapy plans for residents and short-term patients. You will collaborate with an interdisciplinary team, screen patients as needed, and document care in accordance with regulations.

Responsibilities

• Develop and implement a Speech Therapy plan of care for facility residents and short-term patients.
• Assist with patient screens for Speech Therapy as needed.
• Communicate progress with interdisciplinary team members toward SLP goals.
• Complete timely Speech therapy documentation in compliance with regulations.
• Maintain rehab gym equipment and treatment area with proper infection control.
• Provide patient, family, and caregiver training on each plan of care.
• Maintain productivity standards as directed by Rehab Director or Administrator.

This position is for a skilled nursing facility with both short-term and long-term patients. Harbor Point Skilled Nursing has an in-house therapy team with smaller daily case loads, all new equipment, and an emphasis on optimizing time with patients.

Speech Therapist Job Responsibilities:

• Develop and implement a Speech Therapy plan of care for facility residents and short-term patients.

• Assist as needed with patient screens for Speech Therapy.

• Communicate progress and any barriers toward SLP goals with interdisciplinary team members.

• Complete timely Speech therapy documentation in compliance with regulatory, licensing, facility, and payor requirements.

• Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods.

• Provide and document patient, family, and caregiver training regarding each speech therapy patient’s specific plan of care and recommendations for follow-up.

• Maintain productivity standards as expected by the Director of Rehab or Administrator.
