Job summary

Floresville Nursing and Rehabilitation is seeking a Physical Therapist for full-time support in a facility serving short-term and long-term residents in Floresville, TX. The in-house therapy team works with smaller daily case loads to optimize patient time and outcomes.

Texas PT licensure is required; new grads are welcome to apply. Compensation is $48–$52 per hour with comprehensive benefits and flexible scheduling.

Responsibilities

• Develop and implement the Physical Therapy plan of care for facility residents and short-term patients.
• Provide supervision, as needed, for staff Physical Therapy Assistants in delivery of the PT plans of care.
• Assist as needed with patient screenings for the need for Physical Therapy.
• Communicate progress and any barriers toward PT goals with interdisciplinary team members.
• Complete timely PT documentation in compliance with regulatory, licensing, and payor requirements.
• Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods.
• Provide and document patient, family, and caregiver training regarding each PT patient's specific plan of care and recommendations for follow up.
