Job summary

Golfcrest Nursing and Rehabilitation in Houston, under new ownership, seeks a Speech-Language Pathologist to serve short-term and long-term residents.

The role includes developing and implementing a Speech Therapy plan of care, screening patients, and documenting in compliance with regulatory requirements. Must hold Texas SLP licensure; salary starts at $48 per hour.

Responsibilities

• Develop and implement a Speech Therapy plan of care for residents and short-term patients.
• Assist with patient screens for Speech Therapy.
• Communicate progress and barriers toward SLP goals with the interdisciplinary team.
• Complete timely Speech therapy documentation in compliance with regulatory, licensing, facility, and payor requirements.
• Maintain rehab gym equipment and treatment area with infection prevention methods.
• Provide patient, family, and caregiver training regarding each patient’s plan of care and follow-up recommendations.
• Maintain productivity standards as directed by the Director of Rehab or Administrator.

Job description

This position is for a skilled nursing facility under new ownership in Houston. Golfcrest Nursing and Rehabilitation has both short-term and long-term patients. This facility has an in-house therapy team with smaller daily case loads with an emphasis on optimizing time with patients.

Speech Therapist Job Responsibilities:

• Develop and implement a Speech Therapy plan of care for facility residents and short-term patients.
• Assist as needed with patient screens for Speech Therapy.
• Communicate progress and any barriers toward SLP goals with interdisciplinary team members.
• Complete timely Speech therapy documentation in compliance with regulatory, licensing, facility, and payor requirements.
• Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods.
• Provide and document patient, family, and caregiver training regarding each speech therapy patient’s specific plan of care and recommendations for follow up.
• Maintain productivity standards as expected by Director of Rehab or Administrator.
