Job summary

Sweetwater Care seeks a Speech-Language Pathologist to evaluate and treat patients with speech, language, swallowing and cognitive disorders in a skilled nursing facility. You will develop treatment plans and use evidence-based methods to improve communication and safety.

Collaborating with physicians, therapists and team members, you’ll document progress, ensure regulatory compliance, and advocate for patient needs while delivering compassionate, patient-centered care.

Qualifications

• Evaluate and treat speech, language, swallowing and cognitive disorders.
• Develop treatment plans with short-term and long-term goals.
• Collaborate with physicians, therapists and care team.

Responsibilities

• Direct all aspects of quality patient care, including evaluation and treatment, treatment planning, goal setting, family education and documentation.
• Provide professional treatment for rehabilitation of speech, language, hearing, oral motor, swallowing and cognitive disorders.
• Plan activities and provide therapeutic services for speech and language disorders.
• Develop effective treatment plans and obtain approval for services from referring physician.
• Document patient evaluations, treatment progress notes, and discharge summaries.
