Job summary

Synchrony Rehab in Fort Wayne, Indiana seeks a Speech Language Pathologist to deliver a full range of SLP services in an interdisciplinary setting. The role includes assessment, treatment planning, and therapy across communication and swallowing domains.

The ideal candidate will hold a state SLP license, preferably CCC-SLP, and have a Master's degree in Speech-Language Pathology. 0–1 years experience is noted; CPR certification is a plus.

Qualifications

• Must hold a current, valid state SLP licensure.
• CCC-SLP certification from ASHA preferred or required.
• Current CPR certification preferred.
• Master's degree in Speech-Language Pathology from an accredited program.

Responsibilities

• Evaluate patients to identify speech, language, voice, and swallowing disorders through assessments and interviews.
• Develop and implement individualized therapy plans based on assessments with measurable goals.
• Provide therapy to improve communication, articulation, language, voice, fluency, and swallowing.
• Educate patients and families about disorders, therapy techniques, and progress strategies.
• Maintain documentation of evaluations, progress notes, and treatment plans per policies.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
