Trilogy Health Services, LLC is seeking a licensed Speech Language Pathologist to join Synchrony Rehab, supporting multiple health campuses in Indiana. The role focuses on assessment, treatment planning, and therapy within an interdisciplinary team.

You will evaluate and treat speech, language, voice, and swallowing disorders, educate patients and families, document progress, and collaborate across care teams. Active or pending SLP licensure and CCC-SLP are preferred; CPR is a plus.

Qualifikationen

• Must hold an active or pending state SLP license to practice in Indiana.
• CCC-SLP from ASHA preferred or required.
• CPR certification preferred.

Aufgaben

• Evaluate patients to identify speech, language, voice, and swallowing disorders.
• Develop and implement individualized therapy plans with measurable goals.
• Provide therapy to improve communication and swallowing functions.
• Educate patients and families about disorders and therapies.
• Document evaluations, progress notes, and treatment plans per policy.
• Collaborate with interdisciplinary team to ensure comprehensive care.
• Other duties as assigned.
