Job summary

Synchrony Rehab, a subsidiary of Trilogy Health Services LLC, is seeking a licensed Speech Language Pathologist to join our interdisciplinary team in Hanover, IN. The role includes assessment, treatment planning and therapy to support patients’ communication and swallowing needs.

The ideal candidate holds an active or pending SLP state license, and a CCC-SLP is preferred or required. Master’s degree in Speech Language Pathology and CPR certification are recommended, with 0–1 years of experience.

Qualifications

• Master’s degree Speech Language Pathology from an accredited program.
• 0-1 years experience.
• Active or pending state SLP licensure required.
• CCC-SLP from ASHA preferred or required.
• Current CPR certification preferred.

Responsibilities

• Evaluate patients to identify speech, language, voice, and swallowing disorders through clinical assessments and patient interviews.
• Develop and implement individualized therapy plans with measurable goals.
• Provide therapy to improve communication, articulation, language comprehension, voice, fluency, swallowing and feeding.
• Educate patients and families about disorders, therapy techniques and progress strategies.
• Maintain accurate documentation of evaluations, progress notes and treatment plans.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
