Job summary

Trilogy Health Services invites applications for a Speech Language Pathologist at Synchrony Rehab in Madison, IN. The SLP will perform assessments, create treatment plans, and deliver therapy across communication and swallowing domains, within an interdisciplinary team.

The role requires an active or pending SLP license; CCC-SLP is preferred or required, with CPR certification preferred. 0–1 years experience is noted; benefits and ongoing professional growth are provided.

Qualifications

• Active or pending SLP state license required
• CCC-SLP from ASHA preferred or required
• CPR certification preferred

Responsibilities

• Evaluate patients to identify speech, language, voice, and swallowing disorders through clinical assessments and interviews
• Develop and implement individualized therapy plans with measurable goals
• Provide therapy to improve communication and swallowing skills
• Educate patients and families about disorders and therapy techniques
• Maintain accurate documentation of evaluations, progress notes, and plans
• Collaborate with interdisciplinary team for comprehensive care
• Other duties as assigned
