Job summary

Trilogy Health Services, LLC seeks a Speech Language Pathologist in Fort Wayne, IN to provide comprehensive SLP services in an interdisciplinary environment, including assessment, planning, and therapy interventions.

The role requires state licensure and a Master’s in Speech-Language Pathology; CCC-SLP is preferred. You will evaluate, design therapy plans, educate patients and families, document progress, and collaborate with the care team.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• State licensure as a Speech-Language Pathologist.
• CCC-SLP certification from ASHA preferred or required.

Responsibilities

• Evaluate patients to identify speech, language, voice, and swallowing disorders.
• Develop and implement individualized therapy plans based on assessment findings.
• Provide therapy to improve communication and swallowing for daily life.
• Educate patients and families on disorders, therapy techniques, and progress.
• Maintain accurate documentation of evaluations, progress notes, and plans.
• Collaborate with interdisciplinary team to ensure comprehensive care.
• Other duties as assigned.
