Job summary

Victorian Post Acute is seeking a compassionate Speech-Language Pathologist (SLP) to join our in-house therapy team on a PRN basis. The role offers flexible per diem scheduling with opportunities to pick up shifts based on facility needs.

Evaluate and treat residents with communication, cognitive, voice, and swallowing disorders, develop individualized treatment plans, document progress, and collaborate with physicians, nursing staff, and dietitians to achieve optimal outcomes.

Qualifications

• Current California SLP license in good standing.
• CCC-SLP preferred or eligibility under state regulations.
• Strong communication, organizational, and interdisciplinary collaboration skills.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled speech-language pathology services addressing speech, language, cognition, voice, and swallowing disorders.
• Perform clinical swallowing assessments and implement dysphagia management programs.
• Collaborate with physicians, nursing staff, dietitians, rehabilitation professionals, residents, and families to achieve optimal outcomes.
• Complete accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries.
• Participate in interdisciplinary care conferences and discharge planning meetings.
• Educate residents, caregivers, and staff on communication techniques, swallowing precautions, and home exercise programs.
• Maintain compliance with all regulatory, facility, and documentation standards.
