Victorian Post Acute in San Francisco, CA is seeking a compassionate Speech-Language Pathologist (SLP) for a PRN per diem position. You will join our in-house therapy team delivering services across speech, language, cognition, voice, and swallowing disorders with flexible shifts and DOE starting pay.

We welcome new graduates and provide a supportive, collaborative environment with ongoing clinical resources. Join our 90-bed rehab-focused facility dedicated to high-quality patient outcomes.

• Active California SLP license in good standing.
• CCC-SLP preferred or eligible per state regulations.

Responsabilidades

• Evaluate residents and develop individualized SLP treatment plans.
• Provide skilled SLP services addressing speech, language, cognition, voice, and swallowing disorders.
• Perform clinical swallowing assessments and dysphagia management.
• Collaborate with physicians, nursing staff, dietitians, and rehab professionals to achieve optimal outcomes.
• Complete accurate documentation: evaluations, progress notes, and discharge summaries.
• Participate in interdisciplinary teams and discharge planning meetings.
• Educate residents and staff on communication techniques and swallowing precautions.

Key Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled speech-language pathology services addressing speech, language, cognition, voice, and swallowing disorders.
• Perform clinical swallowing assessments and implement dysphagia management programs.
• Collaborate with physicians, nursing staff, dietitians, rehabilitation professionals, residents, and families to achieve optimal outcomes.
• Complete accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries.
• Participate in interdisciplinary care conferences and discharge planning meetings.
• Educate residents, caregivers, and staff on communication techniques, swallowing precautions, and home exercise programs.
• Maintain compliance with all regulatory, facility, and documentation standards.

Qualifications

• Current California Speech-Language Pathologist (SLP) license in good standing.
• CCC-SLP preferred or eligibility as required by state regulations.
• Strong communication, organizational, and interdisciplinary collaboration skills.
