Job summary

Tice Valley Post Acute in Walnut Creek, CA is seeking a full-time Speech Language Pathologist to join our in-house rehabilitation team. You will evaluate patients and implement individualized plans addressing speech, language, cognitive, voice, and swallowing disorders.

Collaboration with a multidisciplinary team is essential to support patient independence and recovery. The ideal candidate will have a CA SLP license or eligibility, CCC-SLP preferred, and a compassionate, organized clinical

Qualifications

• Graduate of an accredited Speech Language Pathology program.
• Current California SLP license or eligibility for licensure.
• CCC-SLP preferred.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide speech, language, cognitive, voice, and swallowing therapy.
• Monitor patient progress and adjust treatment plans as needed.
• Maintain accurate and timely clinical documentation.
• Collaborate with physicians, nursing staff, therapists, and other interdisciplinary team members.
• Educate patients and families regarding treatment goals and strategies.
• Participate in care conferences and discharge planning.
• Maintain compliance with applicable state, federal, and facility requirements.

Compensation: $63-$65 per hour , based on experience
