Job summary

Victorian Post Acute is seeking a Speech-Language Pathologist (SLP) for a PRN, per diem role in our in‑house therapy program. The position offers flexible shifts, collaboration with an experienced rehabilitation team, and clinical resources to support residents with communication, cognitive, voice, and swallowing needs.

Qualifications

• Strong communication, organizational, and interdisciplinary collaboration skills.
• Current California Speech-Language Pathologist license in good standing.
• CCC-SLP preferred or eligibility as required by state regulations.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled speech-language pathology services addressing speech, language, cognition, voice, and swallowing disorders.
• Perform clinical swallowing assessments and implement dysphagia management programs.
• Collaborate with physicians, nursing staff, dietitians, rehabilitation professionals, residents, and families to achieve optimal outcomes.
• Complete accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries.
• Participate in interdisciplinary care conferences and discharge planning meetings.
• Educate residents, caregivers, and staff on communication techniques, swallowing precautions, and home exercise programs.
• Maintain compliance with all regulatory, facility, and documentation standards.
