Job summary

Empath Home Health, part of Empath Health, is seeking a compassionate PRN Speech Language Pathologist to serve patients in north Pinellas County (Clearwater, Palm Harbor, Tarpon Springs and nearby areas).

Responsibilities include OASIS assessments, care planning with the medical team, and patient/family education on swallowing and communication strategies; documentation and coordination are required. Florida-licensed SLP with CCC preferred; prior home health experience is a plus.

Qualifications

• Graduate of an approved/accredited college program that meets the educational requirements for the CCC granted by ASHA (or in process).
• Active Florida SLP license.
• Minimum of 1 year of experience as an SLP; home health care experience preferred.
• Strong clinical assessment, communication, and patient-education skills.

Responsibilities

• Perform initial and ongoing assessments, including OASIS assessments when appropriate, to determine functioning, swallowing, and communication ability.
• Develop a plan of care in consultation with the physician and care team.
• Instruct patients and families on swallowing safety and dietary consistency to maintain nutrition.
• Provide education on strategies to improve language, cognition, and sensorimotor development.
• Prepare and submit clinical notes and progress reports in a timely manner.
• Collaborate with the interdisciplinary team to ensure holistic care.

Empath Home Health , a member of Empath Health is seeking a compassionate PRN Home Health SLP (Speech Language Pathologist) to work with our Home Health team to deliver individualized care to patients in their homes in north Pinellas county (including Clearwater, Palm Harbor, Tarpon Springs , and surrounding areas).

Locations

Home health visits available in north Pinellas county (including Clearwater, Largo, Palm Harbor, Safety Harbor, Dunedin, Tarpon Springs , and surrounding areas). It is very flexible - you choose when & where you want to work!

Schedule

PRN role; 5-15+ home health visits per week. If desired, part-time or even full-time may be available!

What you’ll Do

• Perform initial and ongoing assessments, including OASIS assessments at appropriate time points, to identify the client’s level of functioning, swallowing, and communication ability.
• Develop a plan of care in consultation with the physician and other care team members.
• Instruct patient and families on proper consistency of food to maintain nutritional intake and prevent complications of swallowing disorders.
• Instruct patients and families on using special educational strategies and techniques to improve sensory‑motor development, perception, memory, language, and cognition.
• Prepares clinical and progress notes of services provided, patient activities and condition and submit in a timely manner.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You’ll Need

• Graduate of an approved/accredited college program that meets the educational requirements for a Certificate of Clinical Competence in speech pathology or audiology granted by the American Speech and Hearing Association; or enrolled in a program that meets the educational requirements for certification and in the process of accumulating the supervised experience required for certification.
• Active Florida SLP license
• Minimum of 1 year of experience as a licensed Speech Language Pathologist (SLP); home health care experience preferred
• Strong clinical assessment, communication, and patient-education skills
