Job summary

Empath Home Health is seeking a compassionate PRN Speech-Language Pathologist to deliver at-home evaluations and therapy in the Port Charlotte and North Port area. You will assess swallowing, communication, and cognitive function, and develop individualized plans of care with the medical team.

Applicants must hold an active Florida SLP license and ideally have home health experience. This role offers flexible scheduling and opportunities for growth within our mission-driven Empath Health network.

Qualifications

• Active Florida SLP license is required.
• Home health care experience is preferred.
• CCC-SLP certification or in process indicates eligibility.

Responsibilities

• Perform initial and ongoing assessments including OASIS as appropriate.
• Develop plan of care with physician and care team.
• Instruct patients and families on swallowing safety and nutrition.
• Educate patients on speech, language, and cognitive strategies.
• Prepare progress notes and document services timely.
• Collaborate with interdisciplinary team for holistic care.

Empath Home Health, a member of Empath Health is seeking a compassionate PRN Home Health SLP (Speech Language Pathologist) to work with our Home Health team to deliver individualized care to patients in their homes in Port Charlotte & North Port area.

Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support that assists with daily needs as part of a seamless continuum of care.

Locations:

Home health visits in Port Charlotte & North Port, FL area. If preferred, visits also available in Rotonda West, Englewood, Venice, and surrounding areas. You choose when & where you want to work!

Schedule:

PRN; 5-20+ home health visits per week.

What you'll Do

• Perform initial and ongoing assessments, including OASIS assessments at appropriate time points, to identify the client’s level of functioning, swallowing, and communication ability.
• Develop a plan of care in consultation with the physician and other care team members.
• Instruct patient and families on proper consistency of food to maintain nutritional intake and prevent complications of swallowing disorders.
• Instruct patients and families on using special educational strategies and techniques to improve sensory‑motor development, perception, memory, language, and cognition.
• Prepares clinical and progress notes of services provided, patient activities and condition and submit in a timely manner.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care.

What You'll Need

• Graduate of an approved/accredited college program that meets the educational requirements for a Certificate of Clinical Competence in speech pathology or audiology granted by the American Speech and Hearing Association; or enrolled in a program that meets the educational requirements for certification and in the process of accumulating the supervised experience required for certification.
• Active Florida SLP license
• Home health care experience preferred
• Experience using Homecare Homebase (HCHB) EMR is a plus
